The vermilion border contains free nerve endings, Merkel cells, and Meissner corpuscles in densities exceeded only by the fingertips. The orbicularis oris muscle is supplied by the infraorbital, mental, and buccal branches of the trigeminal nerve. LIP FILLER NUMBING PROTOCOL

    CLEANSE THE PERIORAL AREA

    Mild surfactant cleanse, pat dry. No lip balm, gloss, or moisturizer within 4 hours.

    APPLY THIN EVEN LAYER

    0.5 to 1 g of Ultra to the vermilion border, philtrum, and a small margin onto cutaneous lip. No occlusion needed for lip applications — labial tissue absorbs efficiently.

    WAIT 20 TO 25 MINUTES

    25 minutes for first-time patients or high-anxiety. 20 minutes for repeat patients. Lips will appear slightly blanched from Phenylephrine — this is expected and indicates good penetration.

    REMOVE COMPLETELY

    Wipe with gauze, cleanse with alcohol or saline, dry the skin. Lips must be clean before filler injection begins — residual cream interferes with marking and increases systemic absorption when filler needles breach the skin.

    PROCEED WITH INJECTION

    Peak anesthesia at 30-45 minutes post-application. Anesthetic window 3-5 hours. Phenylephrine vasoconstriction persists 30-60 minutes after cream removal — providing bruising reduction during the injection itself.

What is the best numbing cream for lip filler?+

Why is the lip especially hard to numb?+
The lips have one of the highest densities of sensory nerve endings in the body, supplied by the infraorbital, mental, and buccal branches of the trigeminal nerve. Single-agent OTC products at 4-5% Lidocaine do not reliably block this density. Multi-agent BLT and BLT+ layer three or four anesthetic mechanisms to fully cover the area.
Should I use Pro or Ultra for lip filler?+
Ultra is the preferred formulation for lip filler — the added Prilocaine extends depth and duration. Pro is acceptable for confident repeat patients and works well for cheek, jawline, and other facial filler, but for lips specifically, Ultra provides superior comfort.
Can numbing cream be applied inside the mouth?+
BLT cream is formulated for external skin application and should not be applied to the inner mucosal surface. Mucous membrane absorbs anesthetic much more rapidly than skin, increasing systemic absorption beyond safe topical calculations. Apply BLT to the cutaneous and vermilion lip surface only.
How much do I need per patient?+
0.5 to 1 g per lip filler patient. A 30 g pump covers approximately 30-60 appointments. High-volume injector practices use the 60 g pump with auto-reorder at 60 days. Single-use 5 g packets are available for hygiene-focused workflows.
How long does the numbing last?+
Ultra provides 3-5 hours of clinical anesthesia, which more than covers a standard lip filler appointment (15-30 min injection time) plus the post-procedure window. The extended duration also covers combination treatments such as lip filler plus Botox or perioral threads in the same session.
