MediCaine
UltraQuad-agent BLT+ numbing ointment
The most powerful compound numbing formulation we make. Four anesthetic agents plus Phenylephrine — engineered for the most demanding procedures where failure is not an option.
Four-agent formulation
MediCaine Ultra is our most comprehensive formulation. Four distinct anesthetic agents — Benzocaine, Lidocaine, Tetracaine, and Prilocaine — each with its own onset and duration profile, combined with Phenylephrine for vasoconstriction. This is what you reach for when the procedure demands absolute numbing confidence.
How it works
Benzocaine numbs on contact for instant surface relief. Lidocaine drives deep through tissue layers. Tetracaine provides an extended anesthetic window. Prilocaine adds a fourth mechanism of action for areas that resist traditional triple-agent formulas. Phenylephrine constricts blood vessels to maximize agent concentration at the application site.
Best for
- Extensive laser resurfacing (CO2, Erbium)
- Full-face procedures and large treatment areas
- Patients with high pain sensitivity
- Surgical excisions and biopsies
- Any procedure where maximum numbing is non-negotiable
Ointment only
MediCaine Ultra is formulated exclusively as an ointment. The occlusive ointment base creates a sealed barrier over the skin, maximizing absorption and keeping all four agents concentrated at the application site for the strongest possible effect.

MediCaine Ultra at a glance
Ultra adds prilocaine 8% to the BLT base, giving you four anesthetics in one ointment. It is built for the procedures where standard BLT runs out: lips, thread lifts, long resurfacing sessions and minor surgical work.

Application by procedure
Lip augmentation and lip flip
15 to 20 minutes. Keep it on the vermilion and skin border, away from the mucosa unless your protocol says otherwise.
PDO threads and minor surgical work
20 to 30 minutes, then your usual local technique as needed.
Fractional and ablative laser
15 to 30 minutes per label directions. Treat in sections and stay within a conservative surface area.
Deep chemical peels
Per supervising provider protocol. Remove completely before applying the peel.
Starting points only. Dwell times and amounts are set by the supervising provider. More detail in the dwell time guide and how to use.
Safety
Use the smallest amount and area that does the job, and remove the product completely before the procedure. Systemic absorption rises with large surface areas, occlusion, heat and broken skin. The FDA has warned about serious reactions when compounded topical anesthetics were applied over large areas under wrap. Benzocaine and prilocaine are associated with methemoglobinemia. Screen patients for allergies to local anesthetics, G6PD deficiency, pregnancy and relevant cardiac history, and follow your supervising provider's protocol. This product is dispensed to licensed providers for use under their clinical judgment.
Frequently asked
What is in MediCaine Ultra?
Benzocaine 20%, lidocaine 10%, tetracaine 10% and prilocaine 8%, plus phenylephrine 0.5% as a vasoconstrictor, compounded as an ointment.
Why is Ultra ointment only?
The ointment base holds four actives stably and stays put during longer dwell times and occlusion.
How much does MediCaine Ultra cost?
Ultra runs $80 for 30 g up to $220 for 200 g, and $290 for the 454 g jar, shipping included.
Is Ultra safe for large areas?
Use the smallest area and dwell time that works. Benzocaine and prilocaine carry a methemoglobinemia risk, and every topical anesthetic gets riskier with large areas, heat or occlusion. Ultra is meant for focused, deeper work, not full-body coverage.
Who can buy MediCaine Ultra?
Licensed providers only, with NPI and license verified by a pharmacist before shipping.
